Request vaccination history

Use this service to request your vaccination history.

If you are travelling in under 9 weeks time you can request your full vaccination history from us, which you can then take to a private clinic for your vaccinations.

You can use this service if you:

• are registered at the surgery

We’ll ask you for:

• your first and last name, date of birth, sex, postcode, email and phone number
• if applicable, the details of the person you are completing the form on behalf of
